As Micheal Joffe, implies those who worry that the NHS is too bureaucratic (and therefore seek to cut non-medical staff) are conflating management with administration.... Indeed, one might conclude that there are too many managers & not enough administrators in the health service.... a problem that one would also find in the university sector!
When Thatcher started bringing in hospitality staff to run hospitals it was the beginning of the downward spiral. I distinctly remember arguing with a guy who wouldn't listen as the domestic staff were the first to be downgraded by reducing numbers per ward and reducing the quality of disinfectants they had to work with - to my mind this was the start of MRSA. Disempowering the unions through fear was also a deliberate ploy. Streeting is a smiling assassin cut from the same cloth.
Let's say we don't know what the right balance between medics, admins and managers is.
The regional structure of the NHS should be an opportunity for different regions to try different ideas.
So that we can learn what works and replicate, and also learn what doesn't work and so avoid.
It feels really unintelligent to impose one model across the entire NHS. Not only does one size rarely fit all, it is also inherently risky from the above "engineering" analysis.
A key lesson is that the larger an organisation, the more important it is to maintain its ability to "learn".
And the approach doesn't just apply to the balance between medics-admins-managers .. it can apply to other decisions eg procurement, commissioning GP services, use of pharmacies, solving the dentistry puzzle ...
@rzeta0 and there also needs to be a better mechanism for regional/local successes to be communicated & replicated... it happens with medical procedures it seems but less so organisationally
The so called Internal Market has operated for long enough that it should have been possible to assess whether or not it has achieved improved patient care and financial effectiveness. I am not aware of any major study into this. The days of the old Hospital Administrators are long gone. A cost benefit analysis into the changes in organisations and structures would be highly complex. My view is that a study would struggle to show major gains.
The thing about 'internal markets' or quasi-markets is that it is not a policy based on evidence, it is faith-based economics, and so when it doesn't work, that not the fault of the idea, its because it hasn't been done *properly*!
It is the lack of evidence that is my point. We can all point to examples where it may appear to work or not work, but it is not always easy in a highly complex organisation to justify subjective views. A SoS should have done this 15 to 20 years ago. I think our subjective views are very similar, but that is only part of my critique. Politically politicians love to interfere in the NHS, but usually make matters worse.
@DavidjCrook Yes, I agree; the state of continual interference (in any organisation) just makes things worse - innovations (even less good ones) need time to bed in before the next shift (or instead of the next shift).... the problem with the internal market is it a triumph of (economic) ideology, over the actual experience of running the NHS. Sur bit can & should be improved as an organisation, but as the US shows health care & 'markets' are not in any sense a good fit!
I would like to see NHS survive for generations and provide the very services everyone needs. However as you highlighted, it needs to cut waste, like licencing fees paying to US big techs (use open source Linux, Libre office and others) , start building softwares needed and use it across the country. I've a teacher friend who works with NHS data, and as different NHS bodies uses different software, it becomes difficult for them to transform, convert and cross check data
in terms of licensing fees, the way they are procured (using procurement system), it inflated the price (£10 worth of product turns out to be more than that for some strange bureaucracy of procurement system). And I would like to see home students pay nothing or minimum, why do they have to pay so much for higher education!

Sometimes one just falls off the system, presumably included on a waiting list to be dealt with, that is then not dealt with. My experience is shared by large numbers of patients.
The administrators are, in my experience, competent and dedicated people – but the system is not working. The commonly heard complaint that “there’s too much bureaucracy in the NHS” is wrong. What is needed is a well-managed administrative system, where the chief priority is that somebody is responsible for ensuring coordination, and that gaps are filled, eg when an administrator is on leave, ill, or moves to a different job. The present dire situation also means that nurses and doctors have to add administrative tasks to their already-heavy workload, adding to their stress and burnout.
Probably it is not just the NHS, but a widespread problem....Good administration is undervalued, probably because everybody seems to be against “bureaucrats” – and politicians are often keen to be seen to cut “bureaucracy”.
